Obamacare exchanges face insurer exodus and enrollment drop as enhanced subsidies expire

Cigna announced in late April that it will abandon the Affordable Care Act exchanges in 2027. CVS' Aetna has already stopped offering plans. And a consulting firm projects marketplace enrollment could fall by as much as 26 percent this year.

Fifteen years after its passage, the signature healthcare law of the Obama era is shedding insurers, enrollees, and any pretense that massive federal subsidies built something durable.

The proximate cause is straightforward: Congress refused to renew the enhanced premium tax credits that had propped up enrollment since the COVID era. Without that extra taxpayer money flowing to insurers, the economics of the exchanges are reverting to a harsher reality, one that major carriers apparently want no part of.

The numbers tell a grim story for defenders of the law. A KFF survey published in March found that 80 percent of returning ACA Marketplace enrollees said their 2026 premiums, deductibles, or cost-sharing are higher than last year.

More than half, 51 percent, said costs are now "a lot higher." The Wakely Consulting Group projects marketplace coverage may plummet by up to 26 percent in 2026 compared to average enrollment in 2025, the Daily Caller News Foundation reported.

The subsidy spigot did more than inflate costs. It attracted fraud on a massive scale. The Congressional Budget Office estimated in October 2025 that 2.3 million marketplace enrollees improperly claimed the premium tax credit by intentionally overstating their income that year.

The Affordable Care Act was sold as a permanent fix. What it delivered was a permanent dependency, on subsidies, on mandates, and on the willingness of taxpayers to keep writing checks for a product that cannot sustain itself.

GAO investigators created 20 fictitious identities with fake Social Security numbers and invalid information. Every single one of them got approved for taxpayer-funded subsidies. That's a 100% success rate for fraud.

Even better for the criminals — 18 of those 20 fake identities are still receiving subsidized coverage right now in 2025. The monthly subsidies paid to health insurers on behalf of these phantom enrollees? Over $12,300 per month.

GAO found 66,000 Social Security numbers that had more than one year's worth of subsidized coverage in 2024. But one Social Security number takes the prize for audacity. In 2023, that single SSN was used on applications for over 125 insurance policies — the equivalent of 71 years of health insurance coverage.

How does that happen? Centers for Medicare and Medicaid Services (CMS) doesn't block new applications using the same Social Security number.

Premiums for state enrollees are rising 17 percent, while premiums for others are spiking 30 percent, according to a new analysis of recently released data.

Factors behind the increasing premiums include higher hospital costs and more people using weight loss drugs called GLP-1s, KFF said.

Most Americans are insured through their employers, but 24 million obtained health insurance through a marketplace during the 2025 enrollment period, which ended in January. Seventeen million obtained insurance through Healthcare.gov

The average monthly premium in 2025 was $619 before subsidies, or tax credits, and $113 a month after the credits. The increases do not take into account the impact that the expiration of enhanced subsidies would have, KFF said. Congress approved enhanced subsidies for Obamacare in 2021, and later extended them through the end of 2025.

Democrats want a continuation of the subsidies included in legislation to end the government shutdown, but Republicans have said they will not negotiate on the matter until Congress reopens the government.

The union is the first major Democratic-aligned group to demand that Democrats support the GOP’s proposal of a ‘clean continuing resolution.’

A labor union representing U.S. government employees has called on Democrats in Congress to support a GOP-backed funding bill that would end the government shutdown.

The American Federation of Government Employees (AFGE), a union representing 820,000 employees, published a demand from its president, Everett Kelley, on its website on Oct. 27.

Kelley called on leaders of Congress to “reopen the government immediately under a clean continuing resolution that allows continued debate on larger issues.”

“Both political parties have made their point, and still there is no clear end in sight,” Kelley wrote of the shutdown, which appears to be based on a disagreement between Democrats and Republicans on reauthorizing Affordable Care Act (commonly known as Obamacare) health insurance subsidies.
